We accept most major insurance companies. We accept Medicare patients but we do not treat Medicaid patients. We do not participate in many HMOs. If you have questions about insurance coverage, please contact our office.
- Please give us your insurance information to verify at least 72 hours prior to your first visit or any visit if you have changed insurance policies. Some insurance companies take up to 72 hours to verify benefits.
- If we cannot verify your benefits prior to your visit you may be asked to pay in cash prior to your visit. Please bring your insurance card to your visit.
In order to verify benefits or authorizations Insurance companies usually require that we verify your identity and your signature with your driver’s license or legal picture ID. They often require your social security number as an identification number to process your claims.
If you have changed insurance companies we need a copy of your new insurance card and notification at least 48 hours in advance to verify your new benefits. If you do not provide this information so we can verify your new benefits within an adequate time frame prior to your visit you will be responsible for all charges. If you do not bring a picture ID or driver’s license we may not be able to verify your identity and you will be personally responsible for all charges.
Some insurance policies require primary care physician referrals. If your plan requires a referral you personally will be responsible for obtaining all referrals for each and every visit and procedure prior to your visit. If you are seen without a referral you will be responsible for all fees. Insurance companies do not back date referrals.
We file insurance claims as a courtesy to our patients. As a patient you are financially responsible for all the services we provide to you. You are responsible for any and all amounts not covered by your insurance. We expect payment at the time of service rendered for copayments, co-insurance and deductibles. The amounts quoted are an estimate based on insurance information. If there is any amount above and beyond this, based on what the insurance company actually covers, it will be billed to you. The bill is due on receipt. Biopsy pathology charges, if applicable, are billed by the pathologist to you.
Payment by other Methods
We accept cash, checks, VISA, MasterCard, American Express and Discover for payment. If you find it more convenient than mailing a check, you may pay your bill by using your credit card.
